There are two sets of fatigue penalties
1) changes to event resolution (e.g. italicized numbers, L -> 1b lcf, etc.)
2) adjustments to range plays and DFs
#1 takes effect immediately, once the pitcher BF total exceeds the listed BF fatigue rating on his card
#2 takes effect at the beginning of each new inning *after* the pitcher fatigues, and are cumulative for every additional inning

the cumulative penalties don't kick in until the following inning (i.e. current inning "starts" before the pitcher faces a batter and fatigues)

I'm not sure what you're saying here, are you talking about directions you've seen in someone's MIS? Fatigue and rest have different BF cutoffs, it seems like you're referencing them both here without making a distinction (e.g. "2x BF" only has meaning for rest rating).
