[IBL] Starters fatigue in BF system

Just checking that everyone understands (or whether I misunderstand) the fatigue rules for starting pitchers:  When he goes one above his Starter BF number, he's fatigued, and all penalties that apply start at that point.

(Also:  If a Starter becomes fatigued at the very start of an inning, does that first inning begin the -1 range/+1 DF distance rules?  And they *don't* apply in the inning where fatigue begins, if in the middle of the inning, correct?)

I've seen a number of BFs for starters above what any starter BF ratings (34, 40+) with no mention of fatigue.  The way I understand the rule, you *can* go 2 times the BF number, but all batters after BF are as a fatigued pitcher.
