[IBL] Usage appeals

I forgot to mention regarding usage appeals --

You do not need to send a usage appeal if your player has enough injury
credit to reach 75%.

The only players who will be lost to usage are players who have a positive
value in the "w/Inj 75%" column, or a negative value in the "133%" column
on the team usage page.

Each team's usage page has a URL like this (change the team abbreviation to
yours):
http://almanac.ibl.org/Usage/PHI.html
