Yes, though there is not a rule explicitly covering that exact situation, we have always allowed teams to replace players on their playoff roster who are injured in the regular season.

Yes.  Also, be aware that we had a ruling last year that clarified that in section 3.3.2 (change roster between series) "between series" means between any playoff series regardless of whether the team played.  This was to eliminate any confusion about whether teams could make changes after finding out their opponent.  If we use the broadest definition possible of "between series" that would also include the gap between the end of the regular season and the start of the playoffs, and thus would cover the situation that George is asking about.
