I think that is only for changes that are made during the series, which our current rule emulates:

3.3.3   Playoff Roster In-Series Injury Replacements

A team may (but is not required to) replace a player on its playoff roster 
who is injured during the playoffs and will miss games of a subsequent
series. The injured player who is removed from the active roster will be 
ineligible to be re-added to the active roster for the next series, 
regardless of subsequent injuries.
